SDNB asks that all coaches and student-athletes (and managers) take their temperature before coming to practice/competition and perform a self-screening of COVID-19 symptoms. Staff and students should not attend practice/competition if their temperature is above 100 degrees. Contact your healthcare provider AND the coach AND the Athletic Director if presenting symptoms. If a student or staff member would answer “yes” to either of these two questions, they must not attend practice or competition, and must contact their healthcare provider, coach, and Athletic Director.
In the last 14 days, have you tested positive for COVID-19 or been in close contact with a suspected/confirmed case of COVID-19? Close contact is defined as being within 6ft of a person for a total of 15 minutes or more while not wearing a mask.
In the last 24 hours, have you experienced any of the following symptoms in a way that is not normal to you (i.e. not allergies or related to a pre-existing condition): Congestion/runny nose, Cough, Diarrhea, Fatigue, Fever (100 deg or more), Headache, Muscle/body aches, Nausea/Vomiting, New Loss of Taste/Smell, Shortness of Breath/difficulty breathing, Sore Throat?
